How We Work

EverPath plans, coordinates, and keeps watch over the arrangements already in place for your loved one. Most of the time, we're a coordinator and an extra set of eyes. In North Carolina, case by case and with counsel, we may take on a limited formal role (see below).

We do not provide legal or tax advice. We work closely with your attorney and tax professional.

EverPath's three service domains: Financial and Legal Navigation, Health and Care Plan Monitoring, and Community Connection and Personal Development.

We focus oversight on three connected areas, making sure the financial, legal, and care systems already in place keep working together. We meet families where they are, without adding complexity.

1. Financial & Legal Navigation

We work alongside your attorney and financial professionals to help existing plans — trusts, benefits, and legal documents — keep doing what they're meant to.

  • Plan Monitoring: Track key dates, renewals, and requirements so nothing is missed.
  • Documentation Review: Prepare questions and summaries for meetings with your professionals.
  • Information Relay: Clear updates on any changes or actions needed.

2. Health & Care Plan Monitoring

We stay in regular contact with providers and service teams to help care plans get followed, updated, and met.

  • Care Plan Oversight: Confirm established plans are carried out as designed.
  • Provider Coordination: Communicate with providers and ease transitions.
  • Resource Tracking: Keep families informed of programs and services in their area.

3. Community Connection & Personal Development

We help sustain social connection, skill growth, and community participation that fit your loved one's interests.

  • Local Resource Linking: Find social, educational, and recreational options.
  • Life-Skill Support: Encourage progress already part of an existing plan.
  • Social Opportunities: Introductions to peer groups, clubs, and events.

Fiduciary Options (Case by Case)

In North Carolina, and only when it improves day-to-day stability, EverPath may take on a formal role — case by case, with your attorney's advice and any required court approval.

Representative Payee (SSI/SSDI)

Available when the individual lives independently and wants help with monthly budgeting and resource-limit compliance. In licensed group homes, the provider agency is typically the Representative Payee; there, EverPath coordinates instead.

Trustee / Co-Trustee (limited/administrative)

Considered when a narrow trustee role improves plan maintenance and communication. Requires counsel's advice, conflict checks, and formal appointment documents.

Words We Use

Some of these words come up a lot. Here's what they mean, in plain language.

Fiduciary
Someone legally required to act in your loved one's best interest, especially with money.
Representative Payee
A person Social Security approves to receive and manage someone's monthly benefit for them.
ISP (Individual Support Plan)
The written plan that describes the help and services your loved one receives.
Medicaid Waiver
A Medicaid program that pays for support at home and in the community instead of in an institution.
ABLE account
A savings account for people with disabilities that doesn't reduce their benefits.
Guardianship
A court arrangement that names someone to make certain decisions for an adult.
