How It Works
A simple look at how we work with your family — from the first conversation to ongoing support. Each picture opens full size if you'd like a closer look.
The whole journey
From a first hello to ongoing check-ins, here's the full path. If the legal and financial pieces aren't set up yet, we help arrange those first.
Your first meeting
The first meeting takes about an hour. We go through the intake packet together — you don't fill it out alone — and get to know your loved one's needs.
Reviewing your documents
We help you gather and make sense of the important papers — wills, trusts, guardianship or conservatorship, financial records, and care plans. You don't need everything ready on day one.
What we do, and what it costs
Our services and clear, up-front pricing. Any legal and accounting fees are separate and paid directly to those firms.
Seeing the whole picture
As we work with your family, we're building a secure tool that keeps a simple, living picture of how things are going across seven everyday areas of life — drawn from your loved one's Medicaid Plan of Care, records, and regular check-ins. We show what's going well — and we say so plainly when we're not sure. When something important is missing, that area doesn't show a false “all clear” — it tells you what's missing. When approved services aren't actually happening, we flag it so your team can act.
The cards below are a made-up example for a pretend person — not a real client. They preview the kind of at-a-glance picture the secure tool we're building will keep for you.
-
Health & safety

Medications, check-ups, and safety plans are current and on file.
-
Friends & relationships
On track
Regular contact with family and a steady circle of support.
-
Home & daily living
On track
Stable housing with the right day-to-day help in place.
-
Community & activities
Not enough information yet
We don't yet have enough on file about day programs and getting out. We'll gather it.
-
Services & supports
Not enough information yet
The care plan looks like it's missing an expected support, so we can't call this complete. We'll flag what's needed.
-
Approved vs. delivered services
Needs action
The record shows 120 respite hours approved and 40 delivered. We flag the gap so your care team can follow up.
-
Staying coordinated
On track
Everyone who should be in the loop has heard from us recently.
