How We Work

EverPath provides planning, coordination, and ongoing oversight. In some circumstances—and only when it improves day-to-day stability and long-term plan continuity—we may serve in formal fiduciary roles (e.g., Representative Payee; limited/administrative co-trustee) case-by-case, after conflict checks, with counsel's advice, and subject to applicable law and court approval. In licensed group homes, the provider agency typically serves as Representative Payee; in those cases EverPath coordinates rather than serves in that role.

We maintain regular contact with your loved one's healthcare providers and service teams to help ensure that care plans are being followed, updated when necessary, and meeting intended goals.

  • Care Plan Oversight: Verify that established healthcare and support plans are implemented as designed.
  • Provider Coordination: Communicate with service providers to address concerns and ensure smooth transitions.
  • Resource Tracking: Keep families informed of available programs, benefits, and services in their area.

  • Transitions: Pediatric to adult care, provider changes, or school-to-adult services.
  • Insurance Authorizations: Track requests and outcomes so tasks don't stall.
  • Safety & Crisis Plans: Keep a current plan on file and update after events.

3. Community Connection & Personal Development Support

We help sustain your loved one's social engagement, skill growth, and community participation by connecting them with opportunities that fit their interests and abilities.

Key Supports

  • Local Resource Linking: Identify and recommend suitable social, educational, and recreational activities.
  • Life Skill Support: Encourage and track progress in areas such as budgeting, cooking, and personal organization (when already part of an existing care plan).
  • Social Opportunities: Facilitate introductions to peer groups, clubs, and community events.

  • Peer & Family Supports: Curate opportunities and introductions.
  • Self-Advocacy: Point to trainings and tools for rights awareness.

Fiduciary Options (Case by Case)

Representative Payee (SSI/SSDI)

Available when the individual lives independently and requests support with monthly budgeting, petty cash management, and resource-limit compliance (e.g., monitoring the $2,000 countable-asset limit for SSI). In licensed group homes, the provider agency is typically assigned Rep Payee; EverPath coordinates in those settings.

Trustee / Co-Trustee (limited/administrative)

Considered when a narrowly-tailored trustee role enables better plan maintenance, documentation flow, and communication with all "prime movers" of the estate plan. Requires counsel's advice, conflict checks, and formal appointment documents.
